Getting started is a breeze! Simply use RICH FORM just like you would use Shopify's contact form section. Underneath the surface, you'll discover additional blocks (fields) that enable you to build dynamic forms similar to those found in popular platforms like Typeform and SurveyMonkey.

Checkbox and radio
With RICH FORM you can transform any radio or checkbox into an eye-catching field. To do this, search for the "checkbox" or "radio" block in the RICH FORM section. You have the option to add images or keep the classic element style.
Create checkbox and radio groups
Unlock the real power of RICH FORM by creating checkbox or radio groups. Here's how:
- Add as many checkboxes OR radios as you’d like.
- And make sure they share the same group name.
That's it! RICH FORM will automatically group these fields together in the order they appear allowing you to collect choice-driven information. For example, let's say you want to know someone's favorite 🌈 rainbow color. Add 7 radio blocks (accepting only one value), and assign each block the group name "rainbow". You can enhance it further by adding color-based images or stick with the classic radio 🔘 or checkbox 🔲 button style.

The calendar block allows you to create fields for arrival or departure dates, reservations for restaurants, or appointments. It includes month, day, and year fields, enabling your audience to choose any date up to 5 years in the future. You can format the date based on the country in which you operate.

What's a calendar field without a time field? RICH FORM lets you effortlessly add start and end times formatted in the 24-hour clock.

Custom Liquid
The custom liquid block let's you add custom fields using conditional logic. For example you might want to show a custom field to your customers with a certain tag. You can use this block with simple to complex code and even add scripts that connect to some of the third party services you use.
Patterns
The pattern block allows you to collect information that matches a specific pattern using regular expressions. Don't worry about the technical know-how – simply copy and paste any of these common patterns into the pattern field:
- Postal Code:

Hidden Inputs
Hidden inputs are incredibly useful for adding information to forms meant for your eyes only. Suppose you're running an A/B testing campaign which uses the same RICH FORM fields on different pages. For example one on your homepage and another on a custom landing page. Without leaving your inbox you can see which form was submitted.
